Historical NotesNASA medical certification for spaceflight, issued to Jay Apt. Astronauts were required to undergo an annual physical examination at the Johnson Space Center Flight Medicine Clinic. The card acted as a medical insurance and information card when Astronauts were travelling. This certification card was the last issued to Apt before his retirement from NASA on May 31, 1997.
